About The Mr./Miss Soccer Award

Selection Criteria:

To be nominated, a student-athlete must:

• Have exhibited scholastic excellence (a 3.50 or above cumulative GPA);
• Have demonstrated superb sportsmanship on the soccer field;
• Have contributed to the betterment of his/her community through volunteer works;
• Be a graduating senior;
• Be nominated by his/her IHSSCA member coach;
• Complete the nomination form, with grade point validation and parental consent
• Have the entire nomination form submitted by his/her member coach before the posted deadline.

Application Procedure:

The IHSSCA member head coach must:

• Complete both pages of this form and submit by the posted deadline.
• Gather documentation of Honors, Awards, and Community Involvement.
• Ensure the form has been signed by the Athletic Director verifying the cumulative GPA.
• Ensure the form has been signed by a parent.
